Help Wanted: Care Manager
•Responsible for the development and implementation of the care plan to meet individual needs.
•Evaluate potential clients in their home to assess their needs, such as Home Delivered meals and other services to both English and Spanish speaking clients.
•Evaluates potential clients to identify functional limitations that impede independent living.
•Evaluate the care provider’s ability to provide the care needed.
•Conducts follow up review to determine satisfaction of service delivery.
•Conduct outreach to inform clients and community of services available.
•Provide telephone assessment of caller’s needs, provide referrals to services, and conduct follow-up regularly.
•Complete an AAA Universal Intake on all clients and family caregivers prior to client receiving the service and ensure accuracy.
•Collect, report client/program data and prepare accurate, timely reports while maintaining client files.
•Attend quality assurance meetings and trainings
•Meet all program goals as outlined in grants that support the position
•Develop relationships with community partners through networking
•Present education and information to community as needed
•Maintains confidentiality at all times as outlined in the Welfare and Institution code of the Department of Public Social Services.
•Identify, inform and encourage potential and current clients of existing programs and services.
•Coordinate the provisions of services as required by our Support Service Contract.
•Reassess the client every 6 months and monitor per the care plan
•Acts as an advocate on behalf of the clients and the family.
•Assists clients with paper work deem eligibility for housing, financial assistance and transportation.
•Identify clients who are at risk because of an absence of services.
•Perform other duties assigned.
Skills Required:
•Bachelor’s level social services, gerontology, nursing, other health related field
•Two years experience in the aging or health related field
•Non-profit, and ideally, health or related agency experience
•Excellent written and verbal communication skills
•Experience working with multiple staff members
•Proficiency in Microsoft Word, Windows, and database applications
•Regular and consistent attendance
•Successful background investigation
•Able to work independently with little supervision
•Professional appearance and telephone presence
•Problem-solving skills and good judgment
•Well-organized, self-motivated and ability to multi-task
•California driver license, proof of auto insurance, reliable transportation

Compensation:
$14.00-16.00/Hour + Benefits
